It is not intended as, and should not be relied upon as, medical advice, diagnosis, or treatment.

2. Not a Substitute for Professional Medical Care

The information on the Site does not replace consultation with a qualified, licensed healthcare professional who can evaluate your individual circumstances. 3. No Clinician-Patient Relationship Through the Site

Visiting the Site, reading content, taking the Hormone Symptom Quiz, submitting a contact form, scheduling a Discovery Call, or otherwise interacting with our online materials does not create a clinician-patient relationship between you and Core Function Health or Michele Missakian, MSPA-C. A clinician-patient relationship is established only after a formal Initial Comprehensive Consultation and completion of required intake, informed consent, and HIPAA documentation.

4. The Hormone Symptom Quiz Is Not Diagnostic

The Hormone Symptom Quiz is an educational self-assessment tool designed to help you reflect on common symptom patterns related to hormone, thyroid, adrenal, metabolic, and testosterone-related concerns. Quiz results are not a medical diagnosis, do not establish a clinician-patient relationship, do not replace clinical evaluation, laboratory testing, or a personalized treatment plan, and should be discussed with a qualified healthcare provider before any health decisions are made.

5. In Case of Medical Emergency

If you are experiencing a medical emergency, call 911 immediately or go to your nearest emergency room. Do not use the Site, the Hormone Symptom Quiz, contact forms, or any other online communication channel to report a medical emergency.

6. Practice Scope and Eligibility

Clinical services are provided only to adult patients who are physically located in the State of California at the time of their appointment, in accordance with California medical practice laws and regulations. Patients located outside California are welcome to access educational content on the Site but cannot receive clinical care from Core Function Health. Not all patients qualify for every treatment, prescription, or therapy. Treatment recommendations are individualized and made on a case-by-case basis when deemed medically appropriate and medically necessary by the provider.

7. Physician Assistant Corporation Disclosure

Core Function Health Physician Assistant Corporation is organized and operates under the laws of the State of California, under the medical supervision of a licensed California physician. Michele Missakian has a Master of Science Physician Assistant (MSPA-C) and provides medical services and practices in accordance with California's Physician Assistant Practice Act, California Senate Bill 697 (SB 697), and applicable Medical Board and Physician Assistant Board regulations. 

7A. Physician Assistant Practice Disclosure

Core Function Health Physician Assistant Corporation is a California professional corporation owned and operated by Michele Missakian, MSPA-C, a licensed and nationally certified Physician Assistant. Clinical services, including telehealth consultations, hormone optimization, bioidentical hormone replacement therapy (BHRT), testosterone replacement therapy (TRT), medical weight management, longevity medicine, and functional medicine services, are provided by Michele Missakian, MSPA-C, within the scope of practice authorized under California law.

In accordance with the California Physician Assistant Practice Act and California Senate Bill 697 (SB 697), Michele Missakian, MSPA-C, practices pursuant to a written Practice Agreement with a licensed California physician. This collaborative practice arrangement is maintained in compliance with applicable California laws and regulations governing Physician Assistant practice.

Patients may verify the licensure and credentials of Michele Missakian, MSPA-C, through the California Physician Assistant Board. Information regarding licensed physicians may be verified through the Medical Board of California.

Nothing on this website should be construed as creating a physician-patient or provider-patient relationship until an appointment has been completed, appropriate consent forms have been signed, and care has been formally established.

8. Individual Results Vary

Individual responses to hormone therapy, peptide therapy, medical weight management, and other clinical interventions vary. Results are not guaranteed. Outcomes depend on many factors, including medical history, laboratory findings, lifestyle, adherence to the treatment plan, and individual physiology. Past patient experiences are not predictive of future results.

9. Supplements, Prescriptions, and Off-Label Use

Some therapies discussed on the Site or recommended in the course of clinical care may include nutritional supplements, prescriptions, and medications used in accordance with current clinical evidence and standard of care, including in some cases off-label use as permitted under applicable law and within the prescriber's clinical judgment. Statements regarding nutritional supplements have not been evaluated by the U.S. Food and Drug Administration and are not intended to diagnose, treat, cure, or prevent any disease.

10. Third-Party Services and Products

References to third-party services (including Quest Diagnostics for laboratory testing, Fullscript for professional-grade supplements, and Optimantra for scheduling and records) are provided for patient convenience. Core Function Health is not responsible for the products, services, or practices of any third party. Use of third-party services is governed by those parties' own terms and policies.

11. HIPAA and Health Information

Information you share within an established clinician-patient relationship is Protected Health Information ("PHI") under the Health Insurance Portability and Accountability Act ("HIPAA") and is governed by our separate Notice of Privacy Practices, which you receive when you become a patient. Information submitted through general Site forms or the Hormone Symptom Quiz (before becoming a patient) is governed by our Privacy Policy rather than HIPAA. Please do not submit detailed protected health information through public Site forms, once you are a patient, all clinical communications should occur through the secure portal we provide.
