Visceral Fat: Why Where You Carry Fat May Matter More Than the Number on the Scale

When we talk about weight and health, the conversation often centers around one number: the number on the scale.

But body weight alone doesn't tell the whole story.

Two people can have the same weight, or even the same body mass index (BMI), and have very different body composition and cardiometabolic risk.

One reason is where fat is stored.

Visceral fat, the fat stored deeper within the abdominal cavity around internal organs, is metabolically different from the subcutaneous fat that sits directly underneath the skin. Higher amounts of visceral adipose tissue have been associated with insulin resistance, type 2 diabetes and cardiovascular disease.

That makes visceral fat an important part of the conversation about metabolic health, for both men and women.

Visceral Fat vs. Subcutaneous Fat

Not all body fat is the same.

Subcutaneous fat is stored directly beneath the skin. It's the fat you can typically pinch around areas such as the abdomen, hips or thighs.

Visceral adipose tissue (VAT) is located deeper within the abdominal cavity around internal organs.

Visceral fat is biologically active and is associated with metabolic abnormalities and cardiovascular risk. Importantly, research suggests that evaluating fat distribution can provide information that BMI alone may not capture.


Why BMI Doesn't Tell the Whole Story

BMI can be a useful screening tool, but it cannot tell us how much of someone's weight comes from muscle versus fat, or where that fat is located.

This distinction matters.

An international consensus statement on visceral obesity concluded that BMI alone is not sufficient to properly assess cardiometabolic risk associated with increased adiposity. The authors recommended incorporating waist circumference into clinical assessment because it can provide additional information beyond BMI.

In other words:

The scale can tell you how much you weigh. It cannot tell you where you store fat.

That's one reason a more complete metabolic evaluation may look beyond a single number.


Why Visceral Fat Matters for Metabolic Health

Higher amounts of visceral adipose tissue have been associated with cardiometabolic risk factors and conditions including:

  • Insulin resistance and type 2 diabetes

  • Abnormal lipid profiles

  • Hypertension

  • Metabolic syndrome

  • Cardiovascular disease

This does not mean that having abdominal fat automatically means someone has metabolic disease.

It means that fat distribution is another piece of the overall risk picture.

Metabolic health should be evaluated in context, including family history, blood pressure, glucose regulation, lipid markers, lifestyle, body composition and other individual risk factors.


Visceral Fat Can Matter Even When BMI Doesn't Look Concerning

One of the limitations of relying heavily on body weight is that metabolic risk isn't limited to people who meet BMI criteria for obesity.

People with similar BMIs can have very different amounts and distributions of body fat.

Someone can appear relatively lean, or have a BMI that doesn't immediately raise concern, while still having cardiometabolic risk factors that deserve attention.

Conversely, body weight alone should not be used to assume that someone is metabolically unhealthy.

The goal isn't to label people based on body size.

It's to obtain a more complete picture of metabolic health.


Men and Women Store Fat Differently

Fat distribution differs between men and women, and those patterns can change throughout adulthood.

On average, men tend to accumulate more visceral abdominal fat, while premenopausal women tend to store a greater proportion of fat subcutaneously. However, there is considerable individual variation.

Research from the Framingham Heart Study also suggests that the relationship between visceral fat and cardiometabolic risk may differ by sex.

In 3,482 participants followed for an average of approximately 12.7 years, CT-measured visceral adipose tissue was associated with cardiometabolic and cardiovascular outcomes in both men and women. In women, visceral fat demonstrated stronger associations with several cardiometabolic outcomes than BMI.

This is another reminder that body weight alone doesn't provide the entire metabolic picture.


What Happens to Body Composition Around Midlife?

Body composition isn't static.

Age, physical activity, muscle mass, genetics and hormonal changes can all influence how and where the body stores fat.

This becomes particularly relevant during midlife.

For women, the menopausal transition is associated with changes in body composition and fat distribution, including a tendency toward greater central and visceral adiposity.

Men also experience age-related changes in body composition, including changes in muscle mass and fat distribution.

This is one reason metabolic health becomes increasingly important to evaluate as we get older, even when body weight hasn't changed dramatically.


How Do You Know How Much Visceral Fat You Have?

The most precise assessments of visceral adipose tissue generally involve imaging techniques such as CT or MRI, but these aren't necessary or appropriate as routine screening tests for everyone.

In everyday clinical practice, waist circumference can provide useful additional information about abdominal adiposity and cardiometabolic risk.

Waist circumference isn't a direct measurement of visceral fat, and it shouldn't be interpreted in isolation. But it can provide information that BMI alone misses.

The international consensus on visceral obesity recommends considering waist circumference alongside BMI when assessing adults with overweight or obesity.


Can Visceral Fat Be Reduced?

Yes, and this is where the research becomes particularly encouraging.

Exercise has consistently been shown to reduce visceral adipose tissue.

A systematic review and meta-analysis found that aerobic exercise reduced visceral fat in adults with overweight or obesity even without caloric restriction.

A more recent systematic review and network meta-analysis included 84 randomized controlled trials and 4,836 participants. Aerobic exercise, resistance training, combined aerobic and resistance training, and high-intensity interval training were associated with improvements in visceral adipose tissue, although effects varied by exercise type and population.

Another meta-analysis comparing exercise and pharmacologic interventions found that both were associated with reductions in visceral adipose tissue. Exercise produced a greater reduction in visceral fat relative to the amount of overall weight lost in the trials analyzed.

Changes in metabolic health and body composition aren't always completely reflected by changes on the scale.

Looking Beyond Weight: A More Complete Metabolic Evaluation

When evaluating metabolic health, body weight is only one piece of information.

Depending on the individual, a broader evaluation may consider:

  • Waist circumference and body composition

  • Blood pressure

  • Fasting glucose and hemoglobin A1c

  • Insulin and other markers of glucose regulation when clinically appropriate

  • Standard and advanced lipid markers when appropriate

  • Personal and family cardiovascular history

  • Sleep

  • Physical activity and muscle health

  • Nutrition

  • Hormonal and life-stage changes

No single measurement tells us whether someone is healthy.

The goal is to identify patterns, understand individual risk and determine which areas may be modifiable.

Your weight is one piece of health information.

It isn't the whole story.

Where fat is stored, how much muscle you carry, your metabolic markers, cardiovascular risk factors, lifestyle and family history can provide information that the scale simply cannot.

For both men and women, particularly during midlife, looking beyond weight can provide a more complete picture of metabolic health.
