Strength Training After 40: It's About More Than Building Muscle

When people hear "strength training," they often think about building bigger muscles or changing how their body looks.
But after 40, strength training becomes about much more than appearance.
Maintaining muscle and strength can influence physical function, metabolic health, bone health, independence and healthy aging. Research has also found an association between resistance training and a lower risk of premature death.
That makes strength training relevant not only to athletes, but to men and women who want to remain active, capable and independent as they age.

Why Muscle Becomes Increasingly Important as We Age
Muscle isn't simply something that makes us stronger.
Skeletal muscle plays an important role in movement, physical function and metabolic health. As we get older, maintaining both muscle mass and muscle strength becomes increasingly important.
The encouraging part is that getting older doesn't mean you can no longer become stronger.
A systematic review and meta-analysis of randomized controlled trials found that resistance training significantly improved muscle strength even among adults 75 years and older, including adults over age 80. It also produced increases in whole-muscle size.
In other words: it isn't too late to get stronger.
Strength May Matter as Much as Muscle Size
Having muscle and being strong aren't exactly the same thing.
Muscle mass describes how much muscle tissue someone has, while strength reflects what that muscle can actually do.
That's an important distinction when we're talking about healthy aging.
A large overview of systematic reviews evaluated resistance training across 364 primary studies and more than 382,000 participants. Resistance training was associated with improvements in physical functioning as well as reductions in all-cause mortality and cardiovascular disease incidence.
The goal as we age isn't simply to preserve how our bodies look. It's to preserve what our bodies are capable of doing.
Getting out of a chair. Carrying groceries. Picking something up from the floor. Traveling. Playing with children or grandchildren. Participating in sports. Maintaining balance and independence.
Those are measures of health that don't show up on a scale.
Strength Training and Longevity
One of the most interesting areas of resistance-training research is its relationship with longevity.
A 2022 systematic review and meta-analysis examined resistance training and mortality. Compared with no resistance training, participating in any resistance training was associated with approximately a 15% lower risk of all-cause mortality in the included observational studies.
Resistance training was also associated with lower cardiovascular and cancer mortality in that analysis.
Another systematic review and meta-analysis found that resistance training alone was associated with approximately a 21% lower risk of all-cause mortality compared with no exercise. When resistance training was combined with aerobic activity, the association was even stronger.
There is an important distinction here: these studies demonstrate associations. They don't prove that lifting weights by itself causes someone to live longer. People who resistance train may differ from people who don't in other meaningful ways.
But taken together with evidence showing benefits for strength and physical function, the research provides another compelling reason to make muscle-strengthening activity part of a healthy lifestyle.
Strength Training and Metabolic Health
Muscle also has an important relationship with glucose metabolism.
Skeletal muscle is a major site of glucose disposal, which is one reason resistance exercise is particularly interesting from a metabolic-health perspective.
Evidence in adults with metabolic disease supports this relationship.
A 2025 systematic review and meta-analysis of 19 randomized controlled trials in older adults with type 2 diabetes found that resistance exercise improved HbA1c, fasting glucose, lean mass and muscle strength, although not every metabolic or cardiovascular measure improved.
This doesn't mean strength training replaces appropriate medical treatment for diabetes or insulin resistance. It does reinforce an important concept: building and maintaining muscle is part of metabolic health, not simply fitness.
The Scale Doesn't Tell You How Much Muscle You Have
This is especially important when someone is trying to lose weight.
The number on the scale tells us total body weight. It doesn't tell us how much of that weight is fat, muscle, bone or water.
Two people who weigh exactly the same can therefore have very different body composition and metabolic profiles.
And during weight loss, the goal generally isn't simply to make the number on the scale as small as possible. Maintaining lean tissue and physical function matters too.
That's one reason I prefer to think about weight management in terms of body composition and metabolic health, rather than focusing exclusively on pounds lost.
What About People Using GLP-1 Medications?
This conversation becomes particularly relevant during medical weight loss.
Significant weight loss, whether achieved through lifestyle changes, medications, bariatric surgery or another approach, can involve changes in both fat mass and lean mass.
For someone undergoing medical weight management, the goal shouldn't simply be "how quickly can I lose weight?"
The larger conversation may also include adequate nutrition, protein intake, resistance exercise, physical function and preservation of lean tissue based on the individual's medical circumstances.
GLP-1 medications can be valuable tools for appropriately selected patients, but medication doesn't replace the foundations of long-term health.
Strength Training and Bone Health
Muscle isn't the only tissue that responds to mechanical loading. Bone does too.
A systematic review and meta-analysis examining resistance training in older adults found evidence that resistance exercise can positively influence bone mineral density.
Another systematic review and meta-analysis specifically examining progressive resistance training found that appropriately designed training programs could produce improvements in both muscle strength and bone mineral density in older adults.
This becomes increasingly relevant with age, particularly as maintaining skeletal health becomes a greater priority.
Strength training shouldn't be considered a replacement for appropriate osteoporosis screening or treatment. But physical activity, including appropriately prescribed resistance exercise, can be an important component of a comprehensive approach to musculoskeletal health.

Strength Training Matters for Women After 40
For women, maintaining muscle and strength becomes particularly important through midlife and beyond.
Changes associated with aging and the menopausal transition can affect body composition, while bone health also becomes increasingly important.
But the answer isn't simply to try to weigh less. Preserving muscle, strength, bone and physical function matters.
This is why I don't think healthy aging for women should be reduced to a number on the scale, or even to hormone levels alone.
Hormones can be one piece of the picture. So are nutrition, sleep, cardiovascular health, metabolic health, physical activity and muscle.

Strength Training Matters for Men After 40 Too
The same principle applies to men.
Aging can bring changes in body composition, strength, activity level and metabolic health.
The solution isn't automatically testosterone, and it isn't simply losing weight.
A comprehensive approach should consider lifestyle, resistance exercise, nutrition, sleep, cardiometabolic risk factors, medical history and hormonal health when clinically appropriate.
Maintaining the ability to move, lift, carry and remain physically independent matters regardless of sex.

You Don't Have to Become a Bodybuilder
Strength training doesn't mean you have to spend two hours a day in a gym.
The World Health Organization recommends that adults perform muscle-strengthening activities involving all major muscle groups on two or more days per week, in addition to aerobic physical activity.
Depending on someone's health, experience and goals, resistance training might involve:
Free weights
Weight machines
Resistance bands
Body-weight exercises
Functional resistance exercises
The appropriate starting point will be different for everyone.
Someone who hasn't exercised in years shouldn't necessarily follow the same program as someone who has been lifting weights for decades.
Medical conditions, injuries, osteoporosis, cardiovascular disease and other individual factors may also affect what type of exercise is appropriate.
Cardio or Strength Training? You Probably Don't Have to Choose
This isn't a competition between cardio and weights. Both have value.
In fact, research suggests that combining resistance and aerobic activity may provide particularly strong health benefits. One meta-analysis found that the association with lower all-cause mortality was greater among people participating in both resistance and aerobic exercise than among those performing resistance exercise alone.
A well-rounded approach to healthy aging generally includes both: cardiovascular fitness + muscle strength + mobility + balance + recovery. Not one or the other.
Looking Beyond the Scale After 40
If your goal is to improve your health after 40, progress shouldn't necessarily be measured by weight alone.
Depending on the individual, a broader picture might include:
Muscle strength
Body composition
Waist circumference
Cardiovascular fitness
Blood pressure
Glucose regulation
Lipid and cardiovascular risk markers
Bone health
Mobility and balance
Sleep
Nutrition and protein intake
Energy and physical function
Hormonal and life-stage factors when appropriate
The goal isn't simply to become smaller. The goal is to remain metabolically healthy, physically capable and strong enough to keep doing the things that matter to you.

The Bottom Line
Strength training after 40 isn't just about building bigger muscles.
It's about maintaining strength, metabolic health, bone health, physical function and independence as you get older.
Research consistently supports resistance training as an important component of adult health, and international physical-activity guidelines recommend muscle-strengthening exercise at least twice per week.
You don't need to become a bodybuilder. You don't need to chase a certain number on the scale.
But building and maintaining strength may be one of the most valuable investments you can make in what your body is able to do decades from now.
