Sleep and Metabolic Health: Why Sleep Matters for Men and Women After 40

You can eat well. You can exercise. You can pay attention to your hormones and metabolic health.

But if you consistently aren't getting enough quality sleep, you may be overlooking an important part of the picture.

Sleep isn't simply downtime.

Research has linked sleep duration and sleep quality with metabolic and cardiovascular health, while controlled sleep-restriction studies have demonstrated effects on insulin sensitivity, hunger and energy intake.

How Much Sleep Do Adults Need?

There isn't one perfect number that applies to every person.

Large studies consistently suggest that sleep duration and health have a nonlinear relationship. In a dose-response meta-analysis involving approximately 3.8 million participants, researchers found that cardiovascular and cerebrovascular risk varied with sleep duration, with the lowest risk observed around 7 to 8 hours of sleep.

That doesn't mean everyone needs exactly the same amount of sleep.

It means that chronically getting too little, or unusually long amounts of sleep, can be a signal worth paying attention to.

And duration is only one piece of the picture.

Sleep quality, sleep continuity, sleep-disordered breathing and whether you actually feel restored after sleeping matter too.


Sleep and Metabolic Health

One of the most interesting areas of sleep research involves glucose regulation and insulin sensitivity.

A meta-analysis of 41 randomized controlled trials found that experimental sleep restriction decreased insulin sensitivity while increasing subjective hunger and daily energy intake.

Participants consumed approximately 250 additional calories per day during sleep-restriction conditions in the studies analyzed.

There is also controlled research specifically relevant to women in midlife.

In a randomized crossover trial involving postmenopausal women, several nights of sleep restriction reduced insulin sensitivity as measured by a hyperinsulinemic-euglycemic clamp.

The study was relatively small, so it shouldn't be interpreted in isolation. However, it provides experimental evidence that inadequate sleep can influence glucose metabolism.

This is one reason metabolic health isn't simply about what you eat or what the scale says.

How you sleep may matter too.


Sleep, Hunger and Weight Regulation

Anyone who has had a terrible night's sleep may recognize the experience of feeling hungrier or reaching for different foods the next day.

There is research behind that observation.

Controlled sleep-restriction studies have demonstrated increases in hunger and caloric intake.

But it's important not to oversimplify this as simply a "ghrelin problem" or "leptin problem."

Although individual experiments have reported changes in appetite-regulating hormones following sleep deprivation, a larger meta-analysis did not find strong evidence for consistent changes in average leptin or ghrelin concentrations across studies.

The relationship between sleep, appetite and body weight is more complicated than a single hormone.


Can Getting More Sleep Actually Change Eating Behavior?

Interestingly, randomized clinical trial evidence suggests that it may, at least for some people.

In a 2022 randomized clinical trial, 80 adults with overweight who habitually slept fewer than 6.5 hours per night were assigned to either a sleep-extension intervention or to continue their usual sleep habits.

Participants receiving individualized sleep counseling increased their sleep duration by approximately 1.2 hours per night and consumed approximately 270 fewer calories per day compared with the control group.

Importantly, participants weren't instructed to change their diet or physical activity.

That doesn't mean sleeping longer is a weight-loss treatment.

It suggests that adequate sleep may be another important piece of the metabolic-health picture.

Research also reminds us not to overpromise. A more recent randomized trial found that extending sleep improved sleep health in adults with overweight or obesity and habitual short sleep but did not significantly improve insulin sensitivity or glycemic control during the study period.

Science is rarely as simple as: sleep more, fix your metabolism.

Instead, sleep should be considered alongside nutrition, physical activity, body composition, metabolic health and other individual factors.

Sleep and Your Heart

Sleep is also increasingly recognized as an important component of cardiovascular health.

Large prospective studies and meta-analyses have found associations between abnormal sleep duration, sleep quality and cardiovascular outcomes.

A meta-analysis involving more than 3 million participants examined sleep duration, sleep quality, cardiovascular outcomes and mortality. Poor subjective sleep quality was associated with coronary heart disease, while sleep duration also demonstrated relationships with cardiovascular outcomes.

Other prospective studies involving both men and women have found associations between short and long sleep duration and cardiovascular outcomes.

These studies demonstrate associations. They don't mean that one bad night, or sleep duration by itself, causes heart disease.

But taken together, the evidence supports considering sleep as part of an overall cardiovascular and metabolic-health assessment.


Don't Overlook Sleep Apnea

There's another reason persistent poor sleep deserves attention.

Sometimes the problem isn't simply that you're going to bed too late.

Obstructive sleep apnea (OSA) causes repeated episodes of partial or complete upper-airway obstruction during sleep, which can lead to sleep fragmentation and intermittent drops in oxygen.

The American Heart Association has highlighted the relationship between obstructive sleep apnea and cardiovascular disease. OSA is common among people with certain cardiovascular conditions and can be underrecognized and undertreated.

Possible signs that warrant a conversation with a healthcare professional include:

  • Loud or persistent snoring

  • Witnessed pauses in breathing during sleep

  • Gasping or choking during sleep

  • Excessive daytime sleepiness

  • Morning headaches

  • Persistently unrefreshing sleep

  • Difficult-to-control high blood pressure

You don't have to fit a stereotypical picture of someone with sleep apnea to have it.

Persistent symptoms deserve appropriate evaluation rather than simply assuming you need better "sleep hygiene."


Why Sleep Can Become More Complicated in Midlife

Sleep often changes during midlife, but the reasons aren't identical for everyone.

Stress, work schedules, parenting or caregiving responsibilities, alcohol, medications, physical symptoms and sleep disorders can all contribute.

Hormonal and life-stage changes may also play a role.

For women navigating perimenopause and menopause, symptoms such as hot flashes and night sweats can contribute to disrupted sleep.

Men can also experience sleep problems related to lifestyle, health conditions, medications, changes in body composition and sleep-disordered breathing.

The important point is: poor sleep is a symptom, not a diagnosis.

If someone repeatedly struggles to fall asleep, stay asleep or wake feeling restored, it may be worth asking why rather than simply accepting poor sleep as part of getting older.


What Can Support Better Sleep?

The appropriate approach depends on what is disrupting sleep in the first place.

Foundational strategies may include:

  • Maintaining a relatively consistent sleep and wake schedule

  • Getting regular physical activity

  • Paying attention to caffeine timing

  • Limiting alcohol, particularly close to bedtime

  • Creating a dark, quiet and comfortable sleep environment

  • Reducing stimulating activities immediately before bed

  • Addressing symptoms that repeatedly interrupt sleep

  • Evaluating possible sleep apnea or another sleep disorder when clinically indicated

For chronic insomnia, simply adding supplements isn't necessarily the answer. Identifying the underlying sleep problem matters.


Looking Beyond a Single Lab Value

Sleep is another example of why health shouldn't be reduced to one laboratory result.

Depending on the individual, a broader metabolic and health evaluation may consider:

  • Sleep duration and quality

  • Symptoms of sleep-disordered breathing

  • Blood pressure

  • Fasting glucose and hemoglobin A1c

  • Insulin and other metabolic markers when clinically appropriate

  • Lipid and cardiovascular risk markers

  • Body composition and waist circumference

  • Physical activity

  • Nutrition

  • Medications and supplements

  • Hormonal and life-stage factors

  • Personal and family medical history

The goal isn't to order every possible test. It's to understand the pattern and determine which factors actually deserve attention.

Sleep isn't a luxury, and it isn't simply the time when your body "shuts off."

Sleep duration and quality are associated with metabolic and cardiovascular health, and experimental studies demonstrate that restricting sleep can affect appetite, energy intake and insulin sensitivity.

For men and women, particularly during midlife, sleep deserves a place alongside nutrition, physical activity, metabolic health and cardiovascular risk when looking at the bigger picture.

And if poor sleep persists despite good habits, the next question shouldn't always be "what supplement should I take?"

Sometimes the more useful question is: "why am I not sleeping well in the first place?"
