You are eating well. You are exercising. Maybe you are even taking all the "right" supplements.
But you are exhausted during the day, wired at night, waking up around 3 a.m., struggling with weight around your middle, craving sugar or caffeine, and wondering why your hormones suddenly seem off.
Sometimes the missing piece is not another supplement.
It is stress and recovery.
Cortisol gets blamed for everything from belly fat to insomnia, but cortisol itself is not the enemy. It is an essential hormone that helps regulate your stress response, blood pressure, blood sugar, inflammation, immune function, energy availability and sleep-wake cycle.
The goal is not to eliminate cortisol.
The goal is to support a healthy cortisol rhythm.
And that starts with something much less exciting than the latest supplement: sleep, circadian rhythm, movement, recovery, nutrition, gut health and stress management.
What Is Cortisol?
Cortisol is produced by the adrenal glands as part of the hypothalamic-pituitary-adrenal (HPA) axis, one of the body's primary stress-response systems.
Cortisol normally follows a daily circadian rhythm. It generally:
rises around the time you wake up
increases shortly after awakening
gradually declines throughout the day
reaches lower levels during the evening and early portion of sleep
This morning increase is known as the cortisol awakening response (CAR).
That rise is normal. You actually want cortisol in the morning.
It helps your body wake up, mobilize energy and prepare for the day.
The problem is not simply having "high cortisol." Chronic stress, inadequate sleep, circadian disruption, illness, intense training without adequate recovery and other physiologic stressors can affect regulation of the HPA axis.
And sleep and cortisol influence each other.
Poor sleep can affect stress-system regulation, while activation of the stress system can make restorative sleep more difficult.
The Classic 3 A.M. Wake-Up
One of the complaints I hear frequently is:
"I fall asleep fine, but I wake up around 2 or 3 in the morning and my brain turns on."
There is no single "3 a.m. cortisol diagnosis."
Nighttime awakenings can have many causes, including:
psychological stress
insomnia
alcohol
sleep apnea
temperature changes
pain
medications
perimenopause and menopause
urinary symptoms
circadian disruption
metabolic factors
environmental disturbances
But cortisol and the HPA axis are part of the normal sleep-wake system, and increased physiologic arousal can contribute to insomnia and nighttime awakening.
This is why repeatedly waking during the night deserves a broader evaluation rather than automatically reaching for melatonin, magnesium or another sleep supplement.
Sleep Affects Almost Everything
Sleep is not simply downtime.
It is one of the body's most important recovery periods.
Inadequate or poor-quality sleep has been associated with changes in:
insulin sensitivity
glucose regulation
appetite
hunger and satiety signaling
cortisol regulation
inflammation
mood
cognitive performance
cardiovascular health
exercise recovery
body composition
Sleep restriction can have measurable metabolic consequences, including effects on insulin sensitivity, appetite and energy intake.
So when someone tells me they are doing everything possible to improve their metabolic health but sleeping five hours per night, sleep becomes part of the metabolic conversation.
You cannot separate sleep from hormones and metabolism.
Stress + Sleep Can Become a Loop
Stress can make sleep worse.
Poor sleep can make the body's response to stress stronger.
That can create a cycle:
Stress → Poor Sleep → Impaired Recovery → Greater Stress Reactivity → Poorer Sleep
A systematic review of human studies found that poorer objective and subjective sleep quality can increase HPA-axis reactivity to stress.
This is one reason improving sleep can sometimes improve much more than energy.
You may also notice changes in appetite, cravings, exercise recovery, mood and your ability to handle everyday stress.
What About the "Cortisol Belly"?
"Cortisol belly" is a popular social-media term, not a medical diagnosis.
You cannot look at someone's abdomen and diagnose elevated cortisol.
Abdominal fat accumulation is influenced by many factors, including:
genetics
age
sex hormones
menopause
insulin resistance
overall energy intake
alcohol
physical activity
sleep
medications
underlying medical conditions
However, there is real physiology behind the relationship between stress, cortisol and central fat distribution.
Human studies have demonstrated associations between greater cortisol responses to psychological stress and central fat distribution.
That does not mean cortisol is the sole cause of abdominal weight gain, or that everyone carrying weight around their abdomen has elevated cortisol.
Sleep matters here too.
Insufficient sleep can influence insulin sensitivity, appetite, food intake and other aspects of metabolic health.
So if abdominal weight suddenly becomes more difficult to manage, particularly during midlife, I do not think the answer should automatically be:
"Eat less and exercise more."
The bigger picture matters.
Hormones, Stress and Sleep Are Connected
Hormones do not function independently.
Your reproductive hormones, thyroid hormones, insulin, cortisol and sleep-wake signals interact within a much larger physiologic system.
For women, perimenopause and menopause can add another layer.
Changing estrogen and progesterone patterns can coincide with:
disrupted sleep
nighttime awakening
hot flashes
mood changes
changes in body composition
changes in insulin sensitivity
For men, chronic sleep disruption and metabolic dysfunction can also intersect with reproductive, metabolic and overall hormone health.
This is why evaluating hormone symptoms without asking about sleep, stress, exercise, nutrition and metabolic health can miss important pieces of the picture.
Your Gut Is Part of the Stress Conversation Too
The gut and brain communicate continuously through what is commonly called the gut-brain axis.
Research continues to examine the bidirectional relationship among the intestinal microbiome, sleep, circadian rhythm, immune signaling and the HPA axis.
Microbial metabolites, including short-chain fatty acids and metabolites related to tryptophan, may participate in intestinal barrier function, inflammatory signaling, neuroendocrine signaling and sleep regulation.
At the same time, stress and disruption of normal sleep and circadian rhythms may influence the intestinal environment.
Think of it as an interconnected system:
Stress ↔ Sleep ↔ Gut ↔ Brain ↔ Immune & Neuroendocrine Signaling
This does not mean everyone with poor sleep needs extensive gastrointestinal testing.
But when chronic stress and poor sleep occur alongside persistent bloating, altered bowel habits, food-related symptoms or other gastrointestinal concerns, gut health may deserve a closer look as part of the overall clinical picture.
Can Too Much Strength Training Backfire?
Strength training is one of the most important things we can do for healthy aging.
It helps support:
lean muscle mass
bone health
insulin sensitivity
metabolic health
functional independence
But more is not always better.
Exercise itself is a physiologic stressor. That stress is beneficial when your body has adequate opportunity to recover.
Think:
Training + Recovery = Adaptation
Problems can arise when intense training is layered onto inadequate sleep, insufficient nutrition, psychological stress and inadequate recovery.
Research on overreaching and overtraining demonstrates changes in performance and neuroendocrine stress-response systems, although cortisol itself is not consistently elevated in overtraining.
So the message is not:
"Strength training raises cortisol and causes belly fat."
That would be an oversimplification.
The message is:
Your body needs recovery to adapt to training.
If you are lifting intensely every day while sleeping poorly, constantly sore, unusually fatigued, irritable, losing motivation or watching your performance decline, another intense workout may not be what your body needs that day.
A balanced movement routine can include:
strength training
daily walking
yoga
Pilates
mobility work
lower-intensity activity
adequate recovery days
The right balance will be different for every person.
Morning Sunlight: One of the Simplest Tools We Have
One of the first things you can do after waking up does not come in a bottle.
Go outside.
Light is one of the strongest environmental signals for your circadian system.
Morning light exposure helps reinforce your body's internal clock and supports appropriate timing of the sleep-wake cycle.
Try getting outside relatively soon after waking. If you live near the coast, a walk along the Seal Beach pier or the beachfront at sunrise is a simple, local way to build this habit.
A morning walk combines several foundational habits at once:
Natural light + movement + time away from screens + an opportunity to decompress
And it costs nothing.
Sleep Hygiene: Start With the Foundations
Before building an enormous supplement stack for sleep, look at the basics.

Keep a Consistent Sleep-Wake Schedule
Try to wake up and go to bed at approximately the same times each day. Your circadian system responds to consistency.
Get Morning Outdoor Light
Natural morning light provides an important circadian signal. When possible, step outside rather than relying entirely on indoor lighting.
Dim Things Down at Night
Your brain should receive very different environmental signals at 9 p.m. than it receives at 9 a.m.
Reduce unnecessary bright light as bedtime approaches.
Create a Screen Boundary
Try putting the phone away before bed rather than scrolling until your eyes close.
The issue is not only light. Work emails, social media, news and constant notifications can keep your brain engaged when it should be transitioning toward sleep.
Keep the Bedroom Cool, Dark and Quiet
Your sleep environment matters.
Watch Caffeine Timing
Caffeine can remain active for hours. An afternoon coffee may interfere with sleep in some people even if they can initially fall asleep without difficulty.
Be Careful With Alcohol
Alcohol may make you feel sleepy initially, but it can disrupt sleep later in the night.
Create a Wind-Down Routine
Your nervous system benefits from a transition between doing and sleeping.
That may include:
reading
stretching
breathing exercises
meditation
a warm shower
journaling
gentle yoga
The routine does not have to be complicated.
It needs to be repeatable.
Walk More
Walking is highly underrated.
Not every form of movement needs to be intense to be beneficial.
Walking provides movement without adding the recovery demands of another strenuous workout.
A walk outside can also provide natural light, time away from screens and an opportunity to mentally decompress.
Sometimes the most useful addition to a high-stress lifestyle is not another workout.
It is a walk.
Meditation and Mindfulness Are Not Just Wellness Trends
Mindfulness and meditation have been studied for both stress and sleep.
Research suggests mindfulness-based interventions may improve perceived stress and sleep quality in some people.
You do not need to meditate for an hour.
Start with five minutes.
Consistency is more useful than creating another impossible wellness goal that becomes one more source of stress.
Try Journaling Before Bed
If your body is exhausted but your mind starts reviewing tomorrow's responsibilities the moment your head hits the pillow, try getting those thoughts out of your head before bed.
Write down:
tomorrow's priorities
things you are worried about
unfinished tasks
things you do not want to forget
a few things you are grateful for
Think of it as closing some of the mental tabs that have been open all day.
You Cannot Supplement Your Way Out of a High-Stress Lifestyle
Supplements can sometimes have a role.
But supplements should supplement the foundation, not replace it.
You cannot consistently sleep five hours, live on caffeine, work under relentless stress, train intensely seven days per week and expect a supplement to override all of those physiologic stressors.
Before asking:
"What can I take for cortisol?"
I would rather ask:
How are you sleeping?
How much recovery are you getting?
What does your exercise routine look like?
Are you getting outside in the morning?
Are you eating enough protein and nutrient-dense foods?
Are you giving your brain any time without stimulation?
Are there gastrointestinal symptoms, hormone changes or other medical issues that need to be evaluated?
Those questions often tell us much more.
What About Ashwagandha?
Ashwagandha (Withania somnifera) is commonly marketed as an "adaptogen" for stress, sleep and cortisol support.
There is some human clinical evidence supporting potential benefits. In one randomized, double-blind, placebo-controlled trial of stressed adults, ashwagandha supplementation was associated with improvements in anxiety measures and a greater reduction in morning cortisol compared with placebo.
However:
"Natural" does not mean risk-free.
Ashwagandha may influence thyroid function.
A published case report described a previously healthy woman who developed thyrotoxicosis, excess circulating thyroid hormone, while taking an ashwagandha-containing supplement. Her symptoms resolved and laboratory values normalized after discontinuing it.
This does not establish that everyone taking ashwagandha will develop thyroid problems, but it is an important safety consideration.
I would not recommend automatically starting ashwagandha simply because you feel stressed or believe your cortisol is elevated.
Extra caution and discussion with your healthcare professional are appropriate if you:
have hyperthyroidism or another thyroid disorder
have Hashimoto's disease or another autoimmune thyroid condition
take thyroid hormone replacement or antithyroid medication
have a history of abnormal thyroid laboratory results
have an autoimmune condition
are pregnant or breastfeeding
take medications or supplements that could interact with it
Symptoms associated with excessive thyroid hormone can include:
racing or pounding heart
tremor
anxiety
heat intolerance
increased sweating
unexplained weight loss
diarrhea or increased bowel movements
difficulty sleeping
New or significant symptoms should be medically evaluated.
Ashwagandha may have a role for selected individuals, but it should not be viewed as a harmless over-the-counter fix for "high cortisol."
Before supplementing, it is more important to understand what may be driving the stress response in the first place.
That may mean addressing sleep, circadian rhythm, psychological stress, training load, nutrition, gut health, medications, thyroid function or another underlying medical issue.
When Poor Sleep Needs More Than Sleep Hygiene
Sleep hygiene is important, but persistent sleep disruption should not automatically be blamed on cortisol.
Depending on the individual, evaluation may need to consider:
obstructive sleep apnea
restless legs syndrome
iron deficiency
thyroid dysfunction
perimenopause or menopause
medication effects
alcohol
glucose and metabolic health
anxiety or depression
chronic pain
urinary symptoms
other underlying medical conditions
Loud snoring, witnessed pauses in breathing, gasping during sleep, significant daytime sleepiness, morning headaches or persistent insomnia deserve medical evaluation.
The Foundation Comes Before Optimization
Hormone therapy, medical weight management, supplements and other individualized therapies can be useful when appropriate.
But they work within a body that still needs the basics.
Before trying to "optimize" everything, build the foundation:
Sleep. Morning light. Strength. Walking. Recovery. Protein and nutrient-dense food. Gut health. Stress management. Meaningful downtime.
You do not need to eliminate stress from your life.
That is impossible.
The goal is to give your body regular opportunities to recover from it.
Sometimes improving health is not about doing more.
Sometimes it is about giving your physiology enough space to recover.
