For patients who want to begin with a comprehensive evaluation before committing to a program, the following visits are available individually. (Note: each of these is already included in the 6-Month Comprehensive Program.) ### Initial Comprehensive Consultation · 60 minutes Specifically for BHRT/TRT $350 An in-depth consultation reviewing your medical history, current symptoms, hormone and metabolic health, lifestyle factors, previous laboratory testing, and treatment goals. Includes a personalized discussion regarding recommended baseline laboratory evaluation and potential treatment options. + ### Baseline Lab Review & Personalized Treatment Plan · 40 minutes ### For BHRT/TRT, Thyroid Optimization, Metabolic Health, and Functional Health $200 A detailed review of your baseline laboratory results and development of an individualized treatment plan based on your symptoms, goals, laboratory findings, and medical history. ### Baseline Laboratory Testing $350 ### For BHRT/TRT, Thyroid Optimization, Metabolic Health, and Functional Health ### $180 ### Medical Weight loss Management/Longevity Completed through Quest Diagnostics at a facility near you. Labs ordered by Core Function Health are non-refundable.You do have the option to complete baseline lab testing through your primary care doctor. Specialized testing including at home functional tests are not included.* Follow Up Visits · For all Services $150 Follow-up visits are scheduled as needed to review your progress, adjust your plan, and make sure your care stays aligned with your goals. If you request an additional visit outside the planned schedule, we will review whether it is medically appropriate and help determine the best next step. Not every request will require a separate visit, and any added appointments are arranged based on clinical need. ## HRT/TRT and Medical Weight Loss Programs/Longevity Prefer a complete care program instead of a smaller, more limited approach? This option is designed for patients who want a structured, personalized plan aligned with their clinical needs and long term goals. ##### Programs and Pricing ### 6-Month HRT/TRT Comprehensive Wellness Program $2,900 Six months of structured care A personalized continued care program designed for patients undergoing Hormone Replacement Therapy (HRT) and/or Testosterone Replacement Therapy (TRT) who want ongoing support, monitoring, education, laboratory review, and treatment optimization. #### Program includes: - Initial comprehensive consultation - Optional: Initial Medical Weight Loss Management/Longevity Consult if desired. *medications and monthly refill not included. - Initial baseline laboratory review appointment - All standard baseline and standard follow-up laboratory testing during the first 6 months - Four (4) follow-up appointments - Advanced laboratory review with personalized interpretation - Customized treatment plan tailored to your symptoms, goals, and laboratory findings - Personalized nutrition, lifestyle, and wellness recommendations - Personalized supplement guidance, including what to take, how to take it, and what to avoid - Guidance regarding which specialty laboratory tests may be most beneficial and medically appropriate - Ongoing messaging support between visits for guidance, treatment adjustments, and clinical support - A structured, step-by-step plan to help support long-term progress and treatment success * Note: Cost of supplements, prescribed medications, longevity therapies, specialty laboratory testing, and wellness products are NOT included in program pricing. ##### CONTINUATION CARE ### HRT/TRT Maintenance Fee $150 / mo Ongoing Monthly Care For continuing HRT/TRT patients who have completed the initial 6-month treatment program, achieved relative treatment stability, and want continued medical support, monitoring, and prescription management for therapies such as BHRT, TRT, thyroid management, longevity therapies, and wellness and preventive optimization support. #### HRT/TRT Maintenace Fee after 6 month program includes: - Two (2) follow-up visits per month - Quick phone calls and check-ins - Ongoing treatment monitoring - Prescription management - Continued optimization guidance * Cost of laboratory testing, prescriptions, supplements, longevity therapies, medications, and specialty testing are NOT included. Additional visits or complex care may require separate fees. ##### Fee-for-Service Weight Loss and Longevity Support ### Medical Weight Loss Management & Longevity Therapy Services For patients seeking continued medical weight loss management and/or longevity therapy support without enrollment in the comprehensive HRT/TRT program. *Fee for service #### Getting started: Initial Consultation, $299 30 minutes · Reviewing medical history, goals, symptoms, metabolic concerns, and treatment strategies. *medications are not included Baseline Laboratory Testing — $​180 (one-time) Completed through Quest Diagnostics near you, or you can opt to have completed by your preferred primary care provider. #### What your ongoing medical oversight includes: - Patients who engage medical weight loss or longevity services are entitled to one follow-up visit per month. Additional visits requested by patient are $150 each. - Ongoing weight loss and longevity management support - Medication and treatment monitoring - Guidance regarding nutrition, lifestyle, and wellness strategies - Quick phone calls and check-ins as clinically appropriate are included - Treatment adjustments when medically appropriate - Prescription refills when medically appropriate *Does not include cost of medication or refills * Note: Cost of medications, refills, longevity therapies, supplements, prescriptions, and specialty laboratory testing are NOT included in pricing. ## Specialty laboratory testing Specialty laboratory testing is available at an additional cost when clinically appropriate and may include: DUTCH Testing GI-MAP Nutreval Genova Diagnostics SIBO Testing 4-Point Salivary Cortisol 3x4 Genetics Additional specialty testing is available as indicated. Specialty testing is recommended only when it adds meaningful clinical clarity to your plan. ## What's not included in program pricing So you know exactly what you're paying for, and what to budget separately. Prescriptions Supplements Longevity Therapies Performance and Recovery Optimization Therapy Specialty Labs Wellness Products Treatment recommendations, prescriptions, supplements, and therapies are individualized and prescribed only when medically appropriate and medically necessary. Not all patients will qualify for treatment, prescriptions, or specific therapies. ### Insurance & Payment - Core Function Health is a cash-based specialty wellness practice. - We do not accept health insurance. - We are not contracted with or participating in any insurance plans. - We do not bill insurance carriers. This model allows for a more personalized and comprehensive approach to patient care without many of the limitations commonly associated with insurance-based medicine. ### Prescribing & California Residency Policy - Patients must be physically located within the state of California at the time of their appointment, in accordance with California medical practice laws and regulations. - Prescriptions, supplements, longevity therapies, laboratory testing, and treatment recommendations are individualized and prescribed on a case-by-case basis when deemed medically appropriate and medically necessary by the provider. - Not all patients will qualify for treatment, prescriptions, or specific therapies. ## Frequently Asked Questions Do I have to enroll in a program, or can I do a one-time consultation? ↓ You can do either. The Initial Comprehensive Consultation, Lab Review & Treatment Plan, and Baseline Lab Testing are all available individually. Many patients find that the 6-Month Comprehensive Program is the better value if they want ongoing support, but it's not required. What's the difference between the 6-Month Program and the Maintenance Membership? ↓ The 6-Month Comprehensive Program is for patients beginning HRT/TRT, it includes the initial consultation, baseline labs review, all standard follow-up labs, four 30-minute follow-up visits, and ongoing messaging support over six months. The Maintenance Membership is the next step after completing the 6-Month Program, for patients who have achieved treatment stability and want continued monthly monitoring and prescription management. Can I do the Weight Loss program and the HRT/TRT program at the same time? ↓ The Weight Loss & Peptide Management Program is designed for patients who are not enrolled in the comprehensive HRT/TRT program. If you're interested in both, the 6-Month Comprehensive Program is built to address weight, hormones, peptides, thyroid, and other concerns together as part of an integrated plan. What if I want labs done through my own doctor? ↓ You're welcome to obtain laboratory testing through your primary care provider or specialist if you prefer, for both the HRT/TRT and Weight Loss pathways. Please note that labs ordered through Core Function Health are non-refundable. Do you accept HSA or FSA? ↓ Many patients use HSA or FSA funds toward consultations and lab work where eligible. Eligibility depends on your specific plan, so please confirm with your benefits administrator. What happens after my 6-Month Program ends? ↓ Most patients transition into the Maintenance Membership for ongoing monthly support.
